Enrollment is online through a system provided by Simple™. Or, the employer can use their existing enrollment system, whether electronic or paper-based.
If the Telemedicine program is provided on an employer-paid basis, the cost per month is $5.95 per employee for all family members.
The program is $9.95 per month per employee for all family members on a voluntary basis.
